Price Plan & Ontario Electricity Rebate Declaration Form
Effective November 1, 2019, the Ontario government introduced the new Ontario Electricity Rebate, under the Ontario Rebate for Electricity Consumers Act, 2016
Residential, seasonal, and small business customers, as well as any customer who is using less than 250,000 kilowatt hours a year are by default billed the Regulated Price Plan Time-of-Use (RPP TOU) rates and are automatically eligible for the Ontario Electricity Rebate (OER).
Other commercial customers with a demand above 50 kilowatts and annual consumption above 250,000 kilowatt hours may be eligible to receive the Ontario Electricity Rebate if they meet at least one of the eligibility criteria located in Section B.
Effective May 01, 2023, in accordance with section 3.5 of the Ontario Energy Board’s Standard Supply Service Code, RPP TOU consumers now have the option of being charged tiered prices or ultra-low overnight time-of use prices. Consumers that have an interval meter also have the option of being charged the Hourly Ontario Energy Price (HOEP).
Your account will be adjusted upon receipt of this signed and completed Declaration Form. Any delay in processing as a result of missing or inaccurate information on the form will NOT result in adjustments to previous billings. Do not include units that are metered or billed separately from the account referenced.
At any time after submitting the self-declaration form, you may choose to switch between the RPP TOU, RPP Tiered, RPP Ultra-Low Overnight or HOEP pricing. Requests to change your price plan can be sent by fax, mail or email and should include your account number and your service address. If the request is submitted by a third party, please include a letter of authorization permitting the third party to act on your behalf. If you change your price plan, the change will be effective as of the next actual meter reading.

Part 1: Multi-Unit Complexes
Multi-unit complexes that are predominantly residential, such as condominium and apartment buildings, are eligible for the OER. To qualify, each of the three statements below must apply. Please check the box next to each statement to confirm it applies to your account. If your account relates to a retirement residence or a mobile home park, do not complete this Part 1; skip to Part 3 or Part 4 as applicable.
Part 2: Common Elements of Residential Multi-Unit Complexes such as Condominium or Apartment Buildings
Starting July 1, 2022, accounts for any common elements of a multi-unit complex that is predominantly residential are eligible for the OER. To qualify, each of the three statements below must apply. Please check the box next to each statement to confirm it applies to your account.
Part 3: Retirement Residences
Starting July 1, 2022, accounts for a retirement residence will be eligible for the OER. To qualify, each of the four statements below must apply. Please check the box next to each statement to confirm it applies to your account.
Part 4: Mobile Home Parks
