All fields are required, unless otherwise noted, to enable the preliminary connection assessment. You do not need to commit to any information provided in the EVPCIR — changes can be made when submitting a written connection request.

This preliminary consultation does not include all technical evaluations Renfrew Hydro may perform. After submitting this form, Renfrew Hydro will issue a Preliminary Consultation Report (EVPCR) outlining connection complexity.

Note: Capacity is not reserved with this form — it is only reserved upon execution of the Offer to Connect.

All kVA values should be in alternating current (AC).
